Admission Criteria

1. Developmental disability must be the primary diagnosis and may include all levels such as severe and profound, mild and/or moderate, etc.

2. Must be at least 18 years of age with Medicaid eligibility DD waiver funding or private funding.

3. Developmental disability may be accompanied with developmental, physical, emotional, and mental health disorders along with a need for skilled nursing care (vision, sensory or hearing impairment, seizure disorders, cerebral palsy, down syndrome, diabetes, asthma, all levels of skilled care, and others that fall within the scope of developmental, physical, emotional or mental health needs, etc.). Mental health disorders to include: ADHD, Post-Traumatic Stress Disorder, Depression, Bipolar, Obsessive Compulsive Disorder, Anxiety Disorders, Intermittent Explosive Disorder among other disorders that Oaktree Home, LLC has the resources to manage and keep all individuals safe.

4. Men or women individuals

5. Ambulatory or non-ambulatory with the use of mobility aids such as leg braces, wheelchairs, crutches, canes, walkers, etc.

6. Individuals that display behaviors that is not an immediate threat to themselves or others and the need for behavior management is not more restricted than human rights policies of Oaktree Home, LLC can provide

7. Resolved substance abuse and hypo or hyper function

8. Must be free of any communicable disease and be tuberculosis negative.

9. The individual meets the admission standards and desires to receive services from Oaktree Home, LLC

10. Oaktree Home, LLC can meet the needs of the individual and keep everyone safe
